Clinical Pharmacist

We are seeking an experienced Clinical Pharmacist for a full-time permanent opportunity in Northern New York. This role is ideal for a pharmacist with strong clinical pharmacy experience and prior hospital pharmacy experience who is comfortable supporting patient-centered medication management in a healthcare setting.

The Clinical Pharmacist will be responsible for dispensing medications and pharmaceutical supplies in accordance with established departmental policies, procedures, and professional standards. This position requires strong clinical judgment, attention to detail, and the ability to collaborate effectively with patients, families, medical staff, employees, and management.

Key Responsibilities

• Dispense medications and pharmaceutical supplies according to department standards
• Support safe, effective, and appropriate medication use
• Review medication orders for accuracy, safety, and clinical appropriateness
• Collaborate with medical staff and healthcare teams to support patient care
• Communicate professionally with patients, families, employees, and leadership
• Follow all applicable pharmacy policies, procedures, and regulatory requirements

Requirements

• Active Registered Pharmacist license in New York State required
• Clinical pharmacy experience required
• Prior hospital pharmacy experience required
• 2–3 years of pharmacy experience required
• Pharmacist certification required
• Strong communication, clinical judgment, and attention to detail
