The Wesleyan is hiring Registered Nurses to work PRN (as needed) within our Hospice Community. Essential Job Functions: * Assess hospice patients to identify the physical, psycho social, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations. * Implement/develop/document the plan of care to ensure quality and continuity of care. * Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease. * Provide clinical directions to the Hospice Aide/LVN to ensure quality and continuity of service provided. * Assure continuity of quality patient care delivered with appropriate documentation. * Monitor assigned cases to ensure compliance with requirements of third party payor. * Demonstrate commitment, professional growth and competency. * Promote Agency philosophy and administrative policies. * Perform on-call responsibilities and provide on-call service to patients/families as assigned. * Provide effective communication to patient/family, team members, and other health care professionals Qualifications & Experience: * Two years experience as a Registered Nurse in a clinical care setting, hospice preferred. * Must have current Texas drivers license and auto insurance in accordance with state requirements. * Must have unexpired Registered Nurse license issued by the State of Texas. * Must have a current Cardio Pulmonary Recitation Card (CPR) issued by recognized provider such as American Heart Association or American Red Cross that includes hands-on practice and in-person skills assessment. * Must be aware of CMS guidelines for hospice services; State of Texas (TAC). * Must be able to read and write in English and follow written and verbal instructions in English effectively. * Must have a service-minded approach to all aspects of the job. * Alignment with and enthusiasm about The Wesleyan's culture and core values.
