Responsibilities
+ Carry out the plan of medical care developed for the individuals we support.
+ Reinforce teaching of health related services provided by the RN and observation of the individuals' health care needs.
+ After initial training has been provided by the RN, the LPN will in-service and reinforce teaching to include but not limited to the Exposure Control Plan, Infection Control, oral care, skin integrity, constipation, seizures, vital signs, confidentiality, medical progress notes,
+ Provides input to the RN for formulation and implements the Nursing Process to meet the needs of each individual in the residence they are responsible for.
+ Implementation of the Nursing Service Plan & Nursing Plan of Care
+ Works with RN to ensure that medication certification renewals are scheduled and completed by the RN within the residence they are responsible for.

+ New York State Department of Education licensure as a Licensed Practical Nurse
+ Valid NYS Driver License
+ Long term care experience and experience supporting people with intellectual and developmental disabilities preferred
+ Flexibility of scheduling with the abililty to work evenings and weekends

Responsibilities:
The Medical Secretary is responsible for supporting the attending physician by handing inquiries/correspondence (written and phone-based), working with other teams to ensure films are accounted for and delivered to correct locations, reviewing dictation, and obtaining prior-authorizations when needed. DaVita is seeking a Registered Nurse who is looking to give life in an outpatient dialysis center. You can make an exceptional difference in the lives of our patients and their families dealing with end-stage renal failure or chronic kidney disease.
If you haven't considered Nephrology before, read on as we think that you should. Dialysis Experience is NOT required.

* Long term patient relationships. Build meaningful relationship with patients and their family in an intimate outpatient setting.

**DUTIES AND RESPONSIBILITIES:**
1. Provide donor satisfaction through appropriate interaction and responsiveness to donor needs.
2. Determine donor suitability by evaluating normal healthy donors for plasmapheresis procedures and immunizations that meet the criteria of normal plasma donors in accordance with DCOPs, the FDA, OSHA, CLIA and cGMP and Company policies as well as educational experience.
3. Explain informed consents for plasmapheresis and immunization to donors including a description of the plasmapheresis procedure, hazards, potential adverse reactions, and specific immunization programs.
4. Answer and discuss any donor questions and ensure that the donor is given a clear opportunity to refuse participation.
5. Perform physical examinations for donors and review accumulated data in a timely manner to confirm established donor suitability.
6. Assist in performing the following duties in the Donor Room as necessary: complete plasmapheresis and whole blood procedures; ensure donor safety and monitor for reactions, encourage donor's return visits by maintaining a cheerful and friendly attitude; ensure accurate documentation on the donor chart.
7. Supervise or personally provide for the immediate needs of a donor experiencing an acute, untoward reaction to plasmapheresis or immunization.
8. Administer immunizations following DCOP's and manufacturers insert; monitor responses.
9. As necessary, provide appropriate and confidential counseling to unacceptable donors.
10. Maintain the Center's emergency medical kit and equipment in accordance with DCOP's to ensure that equipment is up to date and functioning properly.
11. Review test results and consult with Center physician on any unusual, abnormal, or elevated test results and non-routine situations in addition to all medical incidents which must also be reviewed by Center physician.
12. Any other duties as required by Supervisor.
**JOB SPECIFICATIONS:**
1. Graduate of a recognized healthcare-related educational program such as a nursing or physician assistant. Emergency medical technicians (EMTs) may be approved if prior experience is at an advanced level of operation.
2. Must have excellent patient/donor assessment skills.
3. Must be able to manage emergency situations in accordance with standard medical care practices and with a calm demeanor.
4. Must be currently licensed in the state in which the center operates.
5. Previous work experience should demonstrate adaptability to changing priorities, excellent attention to detail legible handwriting and the ability to deal effectively with people.
6. Must have excellent communication skills and demonstrate leadership and supervisory skills.
7. Must maintain current and valid licenses and pass medical credential evaluation.
**PHYSICAL REQUIREMENTS:**
1. Ability to sit or stand for long periods of time and work flexible schedules.
2. Ability to tug, lift, and pull up to fifty (50) pounds.
3. Ability to operate computer software including DMS (with training) and Microsoft Office.

Licensed Practical Nurse (LPN) Position Overview:
+ Licensed Practical Nurses (LPNs) are indispensable members of our interdisciplinary team and are charged with managing the daily clinical regimen of each resident and sub-acute patient. Our LPNs enjoy a unique opportunity to gain extensive clinical experience and provide advanced care in a home-like setting. The Licensed Practical Nurse (LPN) is assigned to a team of Nursing Assistants and is responsible for skilled treatments, passing of medications and monitoring the personal care services provided by Nursing Assistants; LPNs are also responsible for reporting care and unusual symptoms of residents to supervisor and staff of next shift. State LPN license required.

(LPN - Licensed Practical Nurse):
+ Administer medications and treatments to assigned residents as ordered by physician and utilizing the five rights as applicable.
+ Direct and monitor the personal care duties and nursing care procedures carried out by Nursing Assistants of the assigned team.
+ Assist Nursing Assistants with direct care of residents, as necessary.
+ Report to Unit Manager/Assistant Unit Manager or Charge Nurse Supervisor unusual symptoms, changes of condition, daily needs, and progress of residents.
+ Follow the Plan of Care for each resident, and monitor the performance of Nursing Assistants in implementation of the care plan.
+ Receive controlled substances from the shift Supervisory Nurse and is responsible for all aspects of the storage and distribution of these medications; ensure proper and safe placement and use of medications stored in medication cart, stock medication cupboards, medication room refrigerator and narcotic cabinets.
+ Document as applicable in the care plans, Nursing Notes, Medical Administration Records, Treatment Records and other required records.

+ Credentials as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a current state license required.
+ LPN Training and work experience in a geriatric setting preferred.
+ Ability to supervise and work well with other personnel required.
+ Ability to communicate well verbally and in writing required.
+ Caring, respectful attitude towards residents required.
+ Physical stamina for constant activity required.
+ Ability to perform tasks to established standards of excellence required.
