Advanced Practice Provider- Primary Care - River Family Health Center Share This Opportunity: Job Title: Advanced Practice Provider- Primary Care - River Family Health Center Location: Alexandria Bay, New York Posted Date: 01/23/2023 River Hospital is seeking a full-time Advanced Practice Provider to join our outpatient services team. This provider will be working in our new Medical Office Building providing Primary Care for all ages in our River Family Health Center. Our APP's have a strong support team of Physicians, specialty providers, 1:1 Nurse to Provider ratio, outstanding clinic management, full access to Hospital ancillary services onsite, and the opportunity to join a facility that was named one of the Top 20 best Critical Access Hospitals in the Nation and only one to be named in New York State. Our facility sits on the banks of the beautiful St. Lawrence River in the Heart of the Thousand Islands region. Job purpose The Provider in River Family Health Center is responsible for providing care to patients who utilize the clinic setting for healthcare. They are responsible for clinical decision-making as it relates to care, treatment and follow-up care provisions, teaches disease prevention and health wellness. The provider is also responsible for participating in and supporting Quality Improvement activities for the institution and the clinic area. Duties and responsibilities CLINICAL PRACTICE: Competency: Provides treatment and care to patients in the River Hospital Convenient Care using established procedure, assessment, planning and intervention evaluation. Expectations: + Provides direct patient care within their scope of practice. + Assesses patients + Performs history & physical + Determines from clinical assessment a diagnosis and coordinates care with physician specialists when needed. + Works with other care practitioners to ensure appropriate education and discharge planning. + Initiates immediate patient intervention when an emergency medical condition is determined. Makes arrangements for the patient to be transferred to the Emergency Department. (May or may not accompany the patient.) + Adheres to policies and procedures for documenting care provided in the setting. + Ensures that documentation is concise, legible and appropriate. + Collaborates with the Administrator / Physician in developing and revising patient care standards, protocols, policies and procedures. + Provides pre-employment evaluation and orders necessary titers and vaccines. Qualifications + Graduate of an accredited Physician Assistant/ Nurse Practitioner school + Graduate of an accredited AMA/AACOM approved medical school. + Board Eligible or Board Certified in a Medical Specialty and/or verified designated training in Emergency Medicine. + Current New York State License/Registration + Current BLS + Completion of all credentialing procedures and requirements.
