POSITION SUMMARY: Functions as a core member of the Meadowlark outreach team that involves the Home Visiting Care Coordinator and the local One Health primary care provider, licensed addiction counselor, nurse care manager and, when available, other mental health providers in the primary care clinic. The peer support specialist is responsible for providing and coordinating warm handoffs and mentoring to patients or potential patient families who are expecting or have children up to age two (2), who are impacted by SUD (substance use disorder) and in need of behavioral health services. The Peer Support Specialist (PSS) also provides experience-based input to guide the implementation of the strategic plan for recruiting participating families to the Sacred Families Program according to the community's unique needs.

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

+ Maintains stable sobriety and be willing to self-identify publicly as being in recovery from a mental health, substance use or addiction diagnosis.
+ Obtains/maintains state certification for Peer Supporters.
+ Understands the concept of recovery as it relates to behavioral health; peer support/recovery coaching; wellness and recovery planning.
+ Facilitates patient engagement and follow-up care.
+ Documents in-person and telephone encounters to report to Care Coordinator.
+ Facilitates referrals for clinically indicated services outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care).
+ Participates in multi-disciplinary, team-based care of patients/clients.
+ Participates in training, reporting, and other requirements of various grant supported programs implemented by One Health and partners.
+ Teaches one section of the Fatherhood is Sacred/Motherhood is Sacred per year.
+ Offers Narcan training as often as needed for community groups and citizens.
+ Adheres to all One Health Policies and Procedures.
+ Participates in performance optimization and risk management activities as required.
+ Cooperates and abides by all elements of One Health's Compliance Program.
+ Provides exceptional customer service and models the mission of One Health when interacting with internal and external stakeholders promoting a positive image for the health center.

MINIMUM JOB QUALIFICATIONS:
Education/Certificates/Licenses:
Required
+ BLS Certification
+ State certification for Peer Supporters (can complete Peer Support 101 and start certification process within first 30 days of employment)
+ Valid driver's license
+ Master Narcan Trainer Certification (can be obtained within 60 days of hire)
+ Certified Trainer for Fatherhood is Sacred/Motherhood is Sacred (can be gained within 90 days of hire)
+ Phoenix Multisport Trauma Informed Group Facilitation Certification (can be obtained within 90 days of hire)
Experience:
